Why most CAPA registers are graveyards

The classic CAPA tracker is a spreadsheet where actions accumulate faster than they close, ownership blurs, and 40–60% of items end their life as 'ongoing' or 'deferred'. The root problem: actions are decoupled from causation. Nobody can rank an action by risk-reduction impact because nobody knows which latent failure it addresses.
