    TRIR (Total Recordable Incident Rate) is calculated as (number of OSHA-recordable incidents × 200,000) ÷ total hours worked. The 200,000 constant represents 100 full-time workers × 40 hours × 50 weeks. A TRIR of 1.0 means one recordable per 100 FTEs per year. US industry averages sit at 2.7 (BLS 2022); world-class programmes achieve below 0.5.

    What counts as an OSHA recordable?

    Under 29 CFR 1904, an injury or illness is recordable if it results in: death, days away from work, restricted work or job transfer, medical treatment beyond first aid, loss of consciousness, or a significant injury diagnosed by a healthcare professional. First-aid-only cases and near-misses are not recordable but should be tracked as leading indicators.

    Why TRIR alone is a poor safety indicator

    TRIR is a lagging indicator — it counts injuries that already happened. It can be gamed (reclassifying cases as first-aid, discouraging reporting) and it correlates weakly with the probability of the next fatality. Best-practice HSE programmes pair TRIR with leading indicators: near-miss reporting rate, GFT drift, CAPA close rate, safety observations per FTE.
